Covenant Membership: New Member Questionnaire


It's a great joy to know that you’re considering becoming an active, participating, Covenant Member of Sound City Bible Church! We’re excited to invite you into a committed expression of what we believe God has called us to as a local church — raise up wholehearted followers of Jesus.

This online questionnaire will assist us in getting to know you, and help prepare for the membership interview where you will meet with one of the elders of the church in order to review your questionnaire answers, affirm that we believe the same essentials of the faith, and ensure that you desire to engage in the same mission with us in the North Puget Sound.

*A few important notes before you begin:
(1) Before filling out this questionnaire, please carefully read through our MEMBERSHIP PACKET which explains in detail what is expected in being a member of Sound City.

(2) You’ll also want to set aside approximately 30-40 minutes to complete this questionnaire in one sitting, though most will likely finish it more quickly.

(3) The questionnaire is meant to be filled out individually by each person seeking Covenant Membership. Even if you’re married, the husband and wife should each complete their own questionnaire. However, when scheduling your interview, married couples are encouraged to schedule the interview together.

Upon consideration of the elder commitments described in the Covenant Membership Packet, do you have any questions or concerns about these commitments that you'd like to discuss in the membership interview? If so list them here. 

Do you affirm the doctrinal statement of Sound City as written in the Covenant Membership Packet?

Please explain your understanding of the gospel in appropriate detail, and why the gospel is good news to you?

How does the good news of Jesus impact other key areas of your life, such as your marriage (or how you date, if single), parenting, your career, management your time and finances, etc.?

How old were you when you trusted Jesus as Savior and gave your life to him as Lord?
Where/how did this happen?
How do you know that you're saved?

Have you been baptized since becoming a believer and submitting your life to the lordship of Jesus?

When and where were you baptized? How old were you when you were baptized? If you have not been baptized, do you desire to do so?
*Please list any other questions you have about baptism here as well, and/or other details about your answer to the previous baptism question.

Do you know your spiritual gifts? If so list them as part of your answer.
How are you currently using your gifts to serve Jesus, his church, others, and for your own joy?
In keeping with Scripture (1 Pe. 4:10-11; Gal. 6:10; Ps. 100:2), are you committed to using your gifts to serving regularly, even if you're not serving right now?

Are you committed to pursuing regular participation in a Community Group to faithfully work at growing together in living our the "one another" passages in the New Testament?

How are you stewarding the finances that God has entrusted to you with regard to debt, savings, giving, etc.? Are you committed to financially supporting the work of this local church? Do you agree with the Bible's general guideline of the tithe ("tithe" by definition means 10%)? Please answer all parts of this question, and explain your answers as necessary.  

Are there any habitual sin issues that would prohibit you from honoring the covenant member commitments that need to be brought into the light, confessed, and repented of? (Please share your answer freely, we are far more concerned with your healing, restoration, and growth in Christ than in judging you harshly.)
