Special Event: Baptism Sign-Up

At Sound City Bible Church, we like to celebrate the fact that God breathes life into dead things. One way we celebrate this amazing truth is through baptism. Baptism is incredibly important to Christians because it was put in place by Jesus himself (Matthew 3:15, 28:18, John 3:22). In Romans 6:3-4, the Apostle Paul explains that our baptism is our identification with Jesus: we go under the waters of judgement to identify with his death, and we come up from the water to identify with his new life. If you have recently become a Christian, or you are a Christian who has never been baptized as a believer, we want to celebrate with you by joining you in the waters of baptism. By submitting this form you are granting Sound City Bible Church irrevocable permission to record, edit, alter, copy, exhibit, publish, distribute or otherwise use your image, visual likeness, portrait, or photograph in all forms and media (including but not limited to publications, websites, catalogues, brochures, books, magazines, photo exhibits, motion picture films, and/or video) collectively referred to as "Works") for (i) the photographic and audio/video resources described above, (ii) marketing, publicity and public relations at Sound City Bible Church; or (iii) for any other lawful purpose.

You agree that all right, title and interest in and to all such Works and any reproductions or derivative work thereof shall be the exclusive property of Sound City Bible Church. You understand that Sound City Bible Church may keep or may use the Works and derivative works now and in the future. You waive any right to royalties or other compensation arising or related to the use of your image, visual likeness, portrait, or photograph in any work. You herby agree to hold harmless and release and forever discharge Sound City Bible Church, its officers, directors, employees, elders, deacons and agents from all claims, demands, and causes of action which you, you heirs, representatives, executors, administrators, or any other persons acting on your behalf or on behalf of your estate have or may have by reason of this authorization.
